Ambiguity

Two records conflict or the event identifier is incomplete.

No silent guess: label the conflict and request review.

Duplicate input

The same update arrives through more than one source.

Deduplicate without deleting evidence; flag material differences.

Access change

A previously available source is no longer authorized.

Fail closed, record the missing dependency and notify the system owner.

Provider failure

A model, connector or destination is unavailable.

Do not emit a partial brief as complete; preserve work for controlled retry or manual fallback.
