Accountable roles
Executives and leaders across operations, production, project delivery, client service, commercial operations and technology who can describe a recurring path.
One operating episode
A proposal handoff, program exception, client output or account task described with sources, roles, consequence, corrections and current measure.
No unsupported inference
Generic opinions, vendor claims without operating evidence, customer-confidential details and observations without a defined episode are excluded from quantified results.

Structured interview
Record company profile, respondent role, operating result, trigger, sources and tools, handoffs, exceptions, current controls, baseline measure, correction behavior and accountable owner.
Evidence rating
Separate directly observed workflow evidence, respondent estimates and unverified interpretation. Store no client-confidential source material in the public study dataset.
Analysis
Report denominators, missingness, range and segment composition. Do not generalize beyond the recruited population. Qualitative themes remain labelled qualitative.
Publication gate
Release results only after a predeclared minimum sample, duplicate review, participant confidentiality check and claims review. Publish the instrument, sample description, limitations and analysis date with the findings.
